Sign in to My Account to let us know if you've changed your first or middle name(s). You'll also need to upload a legal document as proof of your name change.
Sign in to My Account and update your information on your profile page. You'll also need to upload a document confirming your name change.
Another way to let us know of your name change
You can let us know if you change your first or middle name by completing the Declaration of change of name (first or middle name only) form and mailing it to the WorkSafeBC Pension Plan.
You can let us know you've changed your first or middle name by updating your profile information in My Account. If you change your last name, you will need to submit a marriage certificate or legal change-of-name document to the plan.
