Submit a form to waive your beneficiary rights to your spouse's pension

Send us a form that allows you, as the spouse of a plan member, to waive all remaining beneficiary rights should your spouse die after retiring.


Waive your rights to your survivor's benefit

  1. Print Form 2 Spouse’s waiver of 60% lifetime survivor benefit and/or beneficiary rights from a pension plan or annuity after payments start
  2. Complete it in the presence of a witness within 90 days of your spouse's pension effective date; your spouse (the plan member) must not be present when you complete the form
  3. Fill in your contact information as spouse, your spouse's contact information as member/former member and the following information for the plan:
    • WorkSafeBC Pension Plan
      P.O. Box 9460

    • Provincial registration number P085663
  4. Check the box next to Waiver A to waive your 60 per cent lifetime survivor's benefit
    • If the plan member chose a joint life option, select the "Lifetime payments that are ___%" box and enter the percentage (below 60%) that you want to receive
    • If the plan member chose a single life option, you can select the "Payments during the ___-year guarantee period" and enter the number of years you want to be covered over the remainder of the guarantee period if your spouse dies before the end of the guarantee period
  5. If you want to waive your beneficiary rights to your spouse's pension, you can also sign Waiver B at retirement. You can sign Waiver B after your spouse has retired if your spouse chose a single life pension with a guarantee period, the guarantee period hasn't expired and you are currently listed as the beneficiary.
  6. Sign and date the form
  7. Have your witness sign the form
  8. Submit the form to the WorkSafeBC Pension Plan with your retirement application

Eligibility

Complete this form if your spouse (the plan member) is within 90 days of their pension effective date.

If your spouse (the plan member) is still working and you want to waive your rights to their pension benefit, submit Form 4 instead.
