We need to know if you change your first, middle or last name.
We need to know if you change your first, middle or last name. This is to ensure we can conduct your pension business legally.
If you change your first or middle name:
- Complete and print the Declaration of change of name (first or middle) form
- Submit the form to the WorkSafeBC Pension Plan by uploading an electronic copy of the form to My Account or mailing it to us
You can also send us a legal change-of-name document.
If you change your last name:
- Submit a copy of one of the following legal documents to the WorkSafeBC Pension Plan:
- marriage certificate
- legal change-of-name document
